ብዘሀ -ቋንቋ : የአንጎል ጤና እና የመርሳት ህመም

ቋንቋ ለሰው ልጅ የተሰጠ የሒወት ብርሐን ነው። ያለ ቋንቋ ሰው ከሰው ምንም አይነት መስተጋብር አይኖረውም ፤ ቋንቋ አልባ አኗኗር ጨለማ ውስጥ ከመቀመጥ በላይ የሒወት ጨለማነት ያዘለ ነው።

ብዙ ቋንቋ መቻል በአንጎል ጤና ላይ የሚኖረው ሐይል ከፍ ያለ ነው። ሁለትና ከዚያ በላይ ቋንቋ የሚችሉ ሰዎች በአንጎል ቋት አቅማቸው (cognitive capacity ) አንድ ቋንቋ ከሚችሉት በላይ ናቸው። ቋንቋ ለሰው ልጅ አንጎል-ቋት እና የአንጎል ልኬት (Intelligenc Quotient ) መዳበርና መበልፀግ ሁነኛ መሳሪያ ነው።

ቋንቋ ለአንጎል ምግብ እና ውሐ ሆኖ ያገለግላል። ሁለት እና ከዚያ በላይ የቋንቋ ችሎታ ያላቸው የማስታወስ ችሎቸው የዳበረ ነው ። ቋንቋ ማሰላለሰልን ይገነባል። ማሰትታወስ ለመማርና ለእውቀት ቅድመ ሁኔታ ነው። ማስታወስ የማይችል ሊማር አይችልም። መማር የማይችል የሰው ሰው ሆኖ ሊቀጥል አይችልም።

መማር ሲባል ሀ.ሁ..ሂ...እያሉ መቁጠር ብቻ ሳይሆን ካለፈ ልምድ መማር፣ ከተፈጥሮ መማር፣ ከሌሎች መማር፣ ከድርጊት መማር መቻልን ጭምር ነው። ፊደል ሳይቆጥር መማር የሚችል ፍጡር ነው የሰው ልጅ።

አዳዲስ ሐሳቦችን አፍልቆ ለችግሮች መፍትሔ መስጠት መቻል ሰዎችን ከሰዎች ሊለያይ የሚችል ነው። የመፍትሔ ሐሳብ አፍላቂነት በብዘሀ-ቋንቋ ይበለፅጋል። ብዙ ቋንቋ የሚችሉ ችግር ከመፍጠር ይልቅ ለተፈጠረ ችግር መፍትሔ ማመንጨት የሚችል አንጎል ባለቤት ናቸው።

ውሳኔ ሰጪነት በብዘሐ ቋንቋ ይጨምራል። አንድን ጉዳይ ተረድቶ በማገናዘብ አጥጋቢ መልስ መስጠት የሚያስችል ምጡቅ አእምሮ ያሻል። ለዚህ ሁነኛውና ተፈጥሯዊው መንገድ ሰዎች ሁለትና ከዚያ በላይ ቋንቋ እንዲያቁ ማድረግ ነው።

የመጃጀት ህመም(Dementia ) ዋነኛ ምክነያት የሆነውን Alzheimer ችግር መቀነስ የሚያስችል ነው ብዘሐ ቋንቋ ። ብዙሐ ቋንቋን የተላበሱ ሰዎች በሗለኛው ዘመናቸው የሚገጥማቸውን ትልቅ የአንጎል ህመም ማስቀረት ይችላሉ። የመጃጀት ህመም በቀላሉ በቋንቋ መከላከያ መሳሪያ መግታት የሚቻል ነው።

በሀገራችን ብዙ ቋንቋዎች ይገኛሉ። ልጆችን ሁለትና ከዚያ በላይ ቋንቋ እንዲያውቁ ማድረግ የህዝብ ለህዝብ ግንኙነትን ከማጠናከር አልፎ የልጆችን የአእምሮና የአንጎል ጤና መጠበቅና ማጎልበት የሚችሉበት ተፈጥሯዊ መንገድ ነው። ቋንቋን ማወቅ ተፈጥሯዊ ችሎታ ነው። ቋንቋዎች ሁሉ ለሰው ልጅ አኗኗር ማቅለያነትና የሒወት ማሳለጫነት የሚውሉ መግባቢያዎች ናቸው። በቋንቋ ይበልፅጉ የአንጎል ጤናዎን ያስጠብቁ!!

The expansion of postgraduate medical education in Ethiopia is a commendable step toward addressing the severe shortage of specialists. However, the rapid establishment of programs in newly opened universities without adequate infrastructure presents serious challenges to training quality.

For example, in Internal Medicine and Pediatrics residency programs, many trainees from new institutions are required to spend nearly half of their training period at established centers such as St. Paul’s Hospital Millennium Medical College (SPHMMC) or Addis Ababa University (AAU).

While these attachments are vital for their exposure, they also highlight the inadequacy of the home institutions to independently provide comprehensive specialist training.

This dependency undermines the credibility of the newer programs and raises concerns about the competency of their graduates.

Furthermore, learning in a fragmented, “block” or “detachment” format significantly impairs educational continuity, weakens the mentor–mentee relationship, and limits the holistic development that residency training demands.

Medical education, particularly at the postgraduate level, requires consistency, depth of supervision, and long-term patient follow-up—elements that are compromised by this arrangement.

Unless urgent measures are taken, there is a risk of producing specialists who are underprepared for independent practice. This not only endangers patient safety but also dilutes the credibility of our postgraduate training system.

To address this, expansion of programs must be aligned with readiness: minimum infrastructure, adequate faculty, and stable clinical exposure must be guaranteed before approval. New institutions should initially partner with well-established ones under a structured mentorship framework until they are capable of independent training.

In parallel, accreditation processes must be strengthened to enforce quality benchmarks, and investments made in faculty development and training resources.

The ultimate goal should not merely be increasing numbers of specialists, but ensuring that every graduate is competent, confident, and capable of delivering high-quality care. Without prioritizing quality alongside expansion, we risk undermining the very healthcare system we aim to strengthen.

ጥንዶች ጋብቻ ከፈፀሙ በኋላ በቀጣዩ የህይወት ምዕራፍቸው የሚጠብቁት ነገር ልጅ መውለድ ነው። አንድ ሴት ምንም የመውለድ ችግር ሳይኖርባት እና የወሊድ መከላከያ ሳትጠቀም በአንድ ወር ውስጥ የማርገዝ እድሏ 20% ሲሆን 6 ወር ውስጥ 60% እንዲሁም በ1 ዓመት 85% ነው። ለሁለት አመት ቢሞክሩ ደግሞ 95% የማርገዝ እድል አላቸው።

15% የሚሆኑ ጥንዶት በአንድ አመት ውስጥ ምንም አይነት የወሊድ መከላከያ ሳይጠቀሙ ቋሚ የግብረስጋ ግንኙነት እያደረጉ የማርገዝ ችግር ያጋጥማቸዋል። ይህም መካንነት ወይም ያለመውለድ ችግር አጋጥሟቸዋል ተብሎ ህክምና ይጀመርላቸዋል።

የሴቷ እድሜ ከ35 አመት በላይ ከሆነ ለ6ወር ሙከራ አድርጋ እርግዝና ካልተፈጠረ የመካንነት ህክምና መጀመር አለባት።

መውለድ ያለመቻል እጅግ በርካታ ምክንያቶች ሲኖሩት አንዳንዶቹ ወደ 20% የሚሆኑት ይህ ነው የሚባል ምክንያት የላቸውም።

ያለመውለድ ችግር (መካንነት) ዋናዋናዎቹ ምክንያቶች እነሆ

1. ከወንድ በኩል የሚኖር መካንነት (male factor infertility)

እንደ ዓለም ጤና ድርጅት መረጃ መሰረት 20% የሚሆነው ያለመውለድ ችግር (መካንነት) ከወንድ በኩል የሚኖር ነው።ይህም በጭንቅላት ውስጥ የሚመረት ሆርምሞን ማነስ, በዘር ፍሬ ላይ የሚያጋጥም ሰፐርም የማምረት ችግር (primary testicular defect in spermatogenesis), የወንድ የዘር ትቦ መዘጋት, እንዲሁም ምክንያቱ የማይታወቅ የወንድ መካንነት (idiopathic male infertility) ዋና ዋናዎቹ ናቸው።

ከ70-80% የሚሆነው በወንዶች በኩል የሚያጋጥም መካንነት በዘር ፍሬ (testes) በተለያዩ ምክንያቶች ዘር ያለማምረት ችግር የሚመጣ ነው። ይህም በዘር (genetics) ፣ በኢንፌክሽን ፣ በአደጋ፣ ጨረር መጋለጥ፣ ለሙቀት መጋለጥ እንዲሁም የሆርሞን ማነስ ምክንያት ሊሆን ይችላል።

2.በሴቶች ላይ እንቁላል የመለቀቅ ችግር (ovulatory dysfunction)

25% የሚሆነው ያለመውለድ ችግር (መካንነት) ምክንያት የእንቁላል ያለመለቀቅ ችግር ነው። አንዲት ሴት ስትወለድ ከ1-2 ሚሊየን የሚጠጉ እንቁላሎች በኦቫሪ ውስጥ ይኖራሉ። አነዚህ እንቁላሎች እድሜዋ አየጨመረ ሲሄድ እየቀነሱ ይመጣሉ። እርግዝና እንዲፈጠር እንቁላል ከኦቫሪ ወደ ማህፀን ግድግዳ መለቀቅ አለበት ይህ የመለቀቅ ሂደት በተለያየ ሆርሞኖች ቁጥጥር ስር የወደቁ ናቸው። የእነዚህ ሆርሞኖች መዛባት እንቁላል እንዳይለቀቅ ሊያደርጉ ይችላሉ።
PCOS ፣ ከአንጎል የሚመነጭ ሆርሞን ማነስ፣የፕሮላክቲን መብዛት ፣ የታይሮይድ ሆርሞን ማነስ/መብዛት እንደምክንያትነት ይጠቀሳሉ።

3.የማህፀን ቱቦ መዘጋት

በግራና በቀኝ በኩል የሚገኙ እንቁላል አስተላላፊ ትቦዎች በተለያየ ምክንያት ሊዘጉ ይችላሉ። በሰርጀሪ፣ በኢንፌክሽን፣ ከማህፀን ውጭ በሚያጋጥም እርግዝና እንዲሁም በሆድ እቃ ውስጥ በሚያጋጥም ጠባሳና መጣበቅ ሊዘጋ ይችላል።

4.በማህፀን ግድግዳ ላይ የሚያድግ እጢ

በማህፀን ግድግዳ ላይ የሚፈጠር እጢ እንቁላልና ስፐርም ሴል እንዳይገናኙ እንዲሁም እርግዝና ከተፈጠረ በኋላም ምቹ ሁኔታ እንዳይኖር ሊያደርጉ ይችላሉ።

5.የማህፀን አፈጣጠር ችግር

በተፈጥሮ ለሁለት የተከፈለ ማህፀን (septated uterus) ተደጋጋሚ ውርጃ እንዲኖር እንዲሁም ምንም ማህፀን ያለመፈጠር ችግር የመካንነት ችግር ሊያመጡ ይችላሉ።

6.ኢንዶሜትሪዮሲስ (endometriosis)

ጠባሳና መጣበቅን በመፍጠር የማህፀን ትቦ እንዲዘጋ እንዲሁም በኦቫሪ ውስጥ እጢ (endometrioma) በመፈጠር የእንቁላል ማምረት ሂደትን በማወክ መካንነትን ሊያመጣ ይችላል።

7.የዘረ መል (chromosome) ችግር
Turner syndrome, down syndrome, Kallman syndrome, የተባሉ የዘረመል ችግሮች ያለመውለድ ችግር ያመጣሉ።

8.ምክንያቱ የማይታወቅ (unexplained infertility)

ብዙ ምርመራዎች ተደርገውም ምንም ምክንያት ላይገኝ ይችላል።ይህም 20% የሚሆኑት ያለመውለድ ችግር ያለባቸው ጥንዶች ላይ ያጋጥማል።

👍1. ለልጅዎ ቋሚ የእንቅልፍ ሰዓት ያዘጋጁ።

👍2. የመኝታ እና የንቃት ሰዓት በትምህርት ቤት እና ከትምህርት ቤት ውጭ ባሉ ምሽቶች አንድ አይነት መሆን አለበት። ከአንድ ቀን ወደ ሌላ ከ 1 ሰዓት በላይ ልዩነት ሊኖር አይገባም.

👍3. ከመተኛቱ በፊት መኝታ ቦታ ምቹ ያድርጉ። እንደ ቴሌቪዥን መመልከት ወይም የኮምፒውተር ጨዋታዎችን መጫወትን የመሳሰሉ አነቃቂ እንቅስቃሴዎችን ከመተኛቱ በፊት ያስወግዱ።
