و اعراضه ماشية مع ال Cushing بس برضو ممكن ال case يكون Addison's disease، تعمل شنو ؟

طوالي تطلب من الpt يفحصل ال Cortisol.

#الطبيعي: إن مستوى الكورتيزول الصباحي يبقى أعلى من الكورتيزول المسائي:-
١. الكورتيزول الصباحي : 5-23 mcg/dl
(من 5 ل 23 ميك/ديسيلتر)

٢. الكورتيزول المسائي  : 3-13 mcg/dl
(من 3 ل 13 ميك/ديسيلتر)

مثال:-
لقيت الصباحي مثلاً 18
المسائي مثلا 10
كده الاتنين الصباحي والمسائي
Within normal range

• #شاكك في Cushing ح تطلب الكورتيزول الصباحي و المسائي (الاثنين).

مثال:-
الكورتيزول المسائي زي الصباحي (عالي) ، فده معناه
Loss of circadian rhythm
وبالتالي هتشك ف Cushing
عشان كده لو شاكي ف Cushing هتطلب الاتنين مع بعض.

• #شاكي في Addison's disease ح تطلب الكورتيزول الصباحي فقط، المساء ما محتاجه.

ليه الكورتيزول المسائي ما محتاجه؟
لإن كده كده الكورتيزول المسائي بيوصل لمستويات قليله جداً. فبالتالي لو طلبته لحالة شاكك إنها Addison's disease ما ح يفيدك

Pressure Ulcer (bedsore) Staging Guide
#Stage_l:
Intact skin with non-blanchable redness of a localized area usually over a bony prominence. Darkly pigmented skin may not have visible blanching; its color may differ from the surrounding area.

#Stage_II:
Partial thickness loss of dermis presenting as a shallow open ulcer with a red pink wound bed, without slough. May also present as an intact or open/ruptured serum-filled blister. Presents as a shiny or dry shallow ulcer without slough or bruising. This stage should not be used to describe skin tears, tape burns, perineal dermatitis, maceration or excoriation.

#Stage_III:
Full thickness tissue loss, Subcutaneous fat may be visible but bone, tendon or muscle are not exposed. Slough may be present but does not obscure the depth of tissue loss. May include undermining and tunneling. The depth of stage III pressure ulcer varies by anatomical location. Bone/ tendon is not visible or directly palpable.

#Stage_IV:
Full thickness tissue loss with exposed bone, tendon or muscle. Slough or eschar may be present on some parts of the wound bed. Often include undermining and tunneling. The depth of stage IV pressure ulcer varies by anatomical location. Stage IV ulcers can extend into muscle and/or supporting structures (e.g., fascia, tendon or joint capsule) making osteomyelitis possible. Exposed bone/tendon is visible or directly palpable.

#Un_Stageable
Full thickness tissue loss in which the base of the ulcer is covered by slough (yellow, tan, gray, green or brown) and/or eschar (tan, brown or black) in the wound bed. Until enough slough and/or eschar is removed to expose the base of the wound, the true depth, and therefore stage, cannot be dertermined. Stable (dry, adherent, intact without erythema or fluctuance) eschar on the heels serves as "the body's natural (biological) cover" and should not be removed.

#Deep_Tissue_Injury
Purple or maroon localized area of discolored intact skin or blood-filled blister due to damage of underlying soft tissue from pressure and/or shear. The area may be preceded by tissue that is painful, firm, mushy, boggy, warmer or cooler as compared to adjacent tissue.

ح تطلب TSH و معاه Free T4.
☆ #ليه ؟
لأنه مع ال Subclinical hypothyrodism
ح تلاقي ال TSH مرتفع
لكن ال Free T4 بيكون normal

أما مع ال Central hypothyrodism
ممكن
تلاقي ال TSH نورمال
لكن ال Free T4 منخفض

#الخلاصة:
Normal Free T4 + High TSH = Subclinical hypothyrodism
Low Free T4 + Normal TSH = Central hypothyrodism

#الــCentral_hypothyrodism :-
يعني المشكلة ف ال Pituitary gland نفسها

Pituitary insufficiency is: a syndrome characterized by one or more anterior pituitary hormone deficiency.

N.B : in central hypothyrodism , TSH may be #Low or #Normal but Free T4 is low.

#سؤال: طيب ليه بنطلب ال T4 مش ال T3 ؟
طبعاً ال Active هو ال T3
(وليس ال T4) فمع مع ال Hypothyrodism بيزيد ال
Conversion of T4 to T3
maintaining T3 level [ Normal T3 level ]
وبالتالي نقدر نقول ، إن ال
Routine checking of T3 is not recommended if hypothyrodism is suspected
وبالتالي ال T3 ما عنده أي دور ف تشخيص أو متابعة حالات حالات ال Hypothyrodism

بشتكي من : Abd pain و mild distention بالاضافة ل Partial constipation

عملت examination لل abdomen : ال abd soft و ماف guarding او rigidity بس في mild tenderness

طلبت الفحوصات جاءت normal، طلبت ال Abd xray جاءتك [زي ما ظاهر في الصور]

حكاية الــ Metformin :-

حاجات لازم تعرفها عنه:-
١. جرعته تبدأ ب 500 mg و ال max حدنا 2000 (و ما تزيد اكتر من كدا، عشان ما دخل الpt في side effects في غنى عنها )

ال side effects تكلم الpt انه:-
~ ح يعمل لك GIT upset زي Gastritis و diarrhea ( خصوصا اول اسبوع) علشان كده أفضل ياخده بعد يأكل (او مع الاكل).

طب ممكن يدخل الpt في Hypoglycemia سواء الpt اكل بعده او لا ؟
لا ما ممكن او بمعنى تاني ال metformin هو Euglycemic

{بإستثناء حالات قليله لو معاه
Oral hypoglycemics drug}

طب ممكن يدخل الpt في metabolic acidosis ؟
ايوا ممكن بس نادر (حسب كلام ال BNF)

طب ممكن يسبب مشاكل في absorption ل    Vit B12؟
ايوا ممكن عشان كدا بتكتب معاه في #الروشتاRx

1 × od (قبل الفطور)
 
٢. تديهو ك First Line Medication في حالات ال DM Type 2

٣. ممنوع تديهو لو eGFR اقل من 30
او الcreatinine فوق 1.5 للرجال (او فوق 1.4 للنساء)

طب ممكن تديهو في حالات PCO، ليه ؟
لانه بيحسن ال insulin resistance
