Mitral area murmer with axillary radiation..... Mitral regurge
Airtic area murmer with root of the neck radiation......... Aortic stenosis

U may hear the murmer all over the chest so u can diffrentiate by radiation only

In case of mitral regurgr ur comment as the follow.
1st heart sound was muffled
2nd heart sound was audible
Systolic murmer was heared all over the chest with maximaum intensity at mitral area radiating to axilla

In case of aortic stenosis ur comment.
1st heart sound was audible
2nd heart sound was muffled
Systolic murmer was heard all over the chest with maximum intensity in the aortic area radiating to the carotid or say root of the neck

When i examin this gentelman by inspection
There's ni scars,deformity and visible pulsation.
By palpation.
Apex beat was felt at 5th intercostal space at mid clavicular line and i can't distinguish the character.
Theres positive lt para sternal heavu and no thrill.
By auscultation.

Valve lesion

Chest examination.
Start ur examination with introduce ur self to the pt and take permision , explain examination,exposure all upper limb .
Start with inspection.
No scars no chest deformity chest looks symetrical ,ask pt to take deep breath and commnt equal chest movment and chest move with respiration,
Palpitaion.
Do ur chest expantion and do ur tactile focal fremitis .
Percussion.
Auscultation.
Now u have to comment on type of breathing and added sound.

Note
..If u put setescope and u hear ronchi so u comment
on type of breathing vsicular breathing with prolonged expirationn
.... if u hear crepitation so ur comment on type of brathing bronchial breathing.

From previouse we understand that only 2 added sounds u will hear
1- ronchi ur comment here
Deacrese in air entry bilateral and type of breathing vesicular breathing with prolonged expiration.
2- crepitation ur comment here
Decrease in air entry bil with bronchial breathing and crepitation in ( u mention the area where u heared the crepitation.
