Combined Extended Pyelolithotomy and Partial Nephrectomy: A Single-Stage, Organ-Preserving Surgery for Concomitant Staghorn Calculus and Renal Cell Carcinoma

The synchronous presentation of a staghorn calculus and renal cell carcinoma (RCC) is rare but formidable clinical entity and presents a significant surgical challenge, traditionally often necessitating radical nephrectomy.

We report a successful case managed with a single-stage procedure combining extended pyelolithotomy and open partial nephrectomy. This approach achieved complete stone clearance, oncological resection with negative margins, and maximal preservation of renal function, demonstrating the viability of nephron-sparing surgery in this complex scenario.

A 65yrs old male presented with flank pain Of 5 months which is worsening over time. Imaging revealed a complete staghorn calculus occupying the renal pelvis and majority of calyces, alongside a contrast-enhancing 4cm exophytic mass on the posterior mid-pole. Renal function was normal. After detailed patient counseling, the patient was managed with Open Extended pyelolithotomy over stent and partial nephrectomy.

Our patient had an uncomplicated postoperative course.Pathological analysis confirmed clear cell renal cell carcinoma, Fuhrman grade 2, with negative surgical margins.

The combination of extended pyelolithotomy and partial nephrectomy is a technically demanding but highly effective strategy for managing concomitant staghorn calculus and RCC. It obviates the need for multiple procedures or radical nephrectomy, offering patients the dual benefits of complete oncological control and maximal preservation of renal parenchyma.

This case underscores the importance of preoperative planning and surgical expertise in achieving optimal outcomes for complex renal pathology.

Reflections on AI

Iโ€™ve been thinking a lot about AI lately and what it really means for us. For me, AI is not a threat but a remarkable creation that brings simplification to life. It doesnโ€™t replace human effort; instead, it eases it. My role is to guide it, to lead it toward the outcome I need.

From my days in medical school up to my professional life now, AI has been a quiet companion โ€” helping refine my learning, supporting my decisions, and opening doors I couldnโ€™t have reached as quickly alone. In medicine especially, from diagnosis to treatment, its impact is already profound, and I believe this is only the beginning.

Yet, many still feel hesitant, even ashamed, to admit they use AI. But I see it differently: using AI well is a skill in itself. Itโ€™s not about dependency; itโ€™s about mastery. The future will belong not to those who fear new tools, but to those who learn to use them wisely.

The fact that some people feel ashamed to admit they use AI is interesting. In reality, itโ€™s no different from using textbooks, journals, or clinical decision support tools. Every generation of doctors has had tools that transformed their practice โ€” AI is just the newest one

AI will not push us out โ€” it will free us to focus on the deeply human parts of our work: judgment, empathy, and care.

Hemophilia is a rare inherited x linked recessive bleeding disorder predominantly affecting the male population group. I wanted to forward these 5 โ— points to my fellow physicians for their due attention In the advancement of hemophilia care in our country

1.โ— The global prevalence of hemophilia is about 1 in 5,000 to 10,000 live male births, with Hemophilia A being the most common form, affecting approximately 1 in 5,000 live births.

Despite this prevalence data, the number of patients diagnosed and treated for hemophilia in our country is less than 600 (out of an expected 6000+ cases in the country). Just like the other African countries, significant number are assumed to be none diagnosed; and hence the low prevalence. More than 80% of these patients on follow up have already developed target joint and limitation of movement; (an earlier study) and this is related to late diagnosis and referal.

While more than 70% of our patients had an evident past family history, most stayed for years with symptoms prior to diagnosis. We have to early work up individuals with a personal or family history of unexplained joint pain, easy bruising and surgical bleed for a possible early referral and management.

2. โ— We have to make it customary to include bleeding history in to our presurgical and pre circumsision evaluation.

Significant number of patients with hemophilia had excess bleed during circumsision and this happens despite the fact that they had one or more older brothers that they have lost or had a life threatening bleeding during a prior circumsision. We believe this could potentially be avoided with proper presurgical evaluation. The same is true for other major surgeries, including abdominal surgery, as we have encountered patients with hemophilia post laparotomy. ( intra abdominal hematomas confused with tumors; and acute abdomen like features of hemophilia confused with appendicitis ).

We also have seen patients with joint bleed for whom Arthrocentesis was tried (adding fuel to the fire ) for suspicion of septic arthritis (while patients clearly had prior recurrent similar symptoms.

3. โ— Not every recurrent bleeding in males that feels and smells like Hemophilia is in fact a case of Hemophilia. We have encountered patients with recurrent joint, post circumsision and surgical bleeding being managed as cases of hemophilia to have an alternative diagnosis including Severe von willebrand disease. Hence, especially in the presence of significantly affected female family members, thorough evaluation and work up is needed before labeling patients as such.

4. โ— For many years, people believed that only men and boys could have symptoms of hemophilia, such as bleeding in general and bleeding into joints, and that women who โ€œcarryโ€ the hemophilia gene do not experience bleeding symptoms themselves.

Even if the circumstances where a women's two X chromosomes can both become defective and lead to a state of moderate to severe Hemophilia is extremely rare, we have to bear in mind that carrier states are more common and can lead to symptomatic cases in women.

According to a report from WHF, third of women with carrier states can have factor activity level less than 40% and can exeperiance symptoms including; post surgical and post partum bleeds, recurrent epistaxis, easy bruising, excessive menstrual bleeding and severe menstrual cramps.Hence, in the absence of any alternative explanation for those symptoms in women of any age group, work up for a possible case of "symptomatic hemophilia carrier" or "mild hemophilia" is recommended.

5.โ— In collaboration with World federation of hemophilia and the Ethiopian hemophilia society, Tikur Anbessa specialized hospital is providing comprehensive hemophilia care; including diagnosis, treatment, education, social support and followup of patients. Bullet Embolization to the Heart โ€“ A Very Rare Case

A 35-year-old man survived a penetrating thoracoabdominal gunshot injury five months ago,but the bullet did not remain where it landed. Instead, it found its way into the venous circulation, traveling silently through the bloodstream before lodging in the right ventricle of the heart.

This exceptionally rare event, known as bullet embolization, occurs when a projectile enters the vascular system after penetrating trauma and migrates with blood flow. Though uncommon, it carries significant clinical risks and demands a high index of suspicion.

With the working diagnosis of a bullet embolization to the right ventricle, the patient underwent on pump bullet removal.

Intraoperatively, the bullet was found embedded in the right ventricle at an acute angle, surrounded by fibrotic tissue.

The postoperative course was uneventful, and the patient was discharged home in stable condition.
