Management of Chronic Gout part 1..

هل أي مريض أجاله Gout flare هيمشي ع أدوية النقرس ولا مش شرط ؟؟ 🤔
When to Initiate Urate-lowering Therapy (ULT) ??

🔸️ متى هيكون في Indication لل ULT :
1️⃣ لو المريض تعرض ل Gout flare مرتين او أكثر خلال السنة

2️⃣ لو المريض عنده Gouty tophi او Radiological damage :
In acase of polyarticular disease ( gouty bone erosion , Tophaceous deposits in soft tissue/ subchondral bone )

3️⃣ For patients experiencing their first flare and CKD stage >=3, SU >9 mg/dl, or urolithiasis, we conditionally recommended initiating ULT ..

4️⃣ very early onset of symptoms

⚠️ أما لو الشخص تعرض ل One flare وما عنده أي Complication ،، بهلحالة ما بنبدأله ULT لان 70% من المرضى اللي هتجيهم ONE Flare مش هتتكرر تاني ...

⚪️⚪️⚪️⚪️⚪️⚪️⚪️⚪️⚪️⚪️⚪️⚪️⚪️⚪️⚪️⚪️⚪️⚪️

ال Target اللي بدنا نوصله من ال ULT هو اننا ننزل
بال Serum uric acid (SUA) لأقل من 6mg/dl ,
ولو المريض كان عنده Tophi فال Target نوصله لأقل من 5mg/dl ..

🔸️ ال SUA بعد 2-4 اسابيع من بداية ال ULT
لو م وصلنا لل Target بنبدأ نعمل Dose titration او نزبط خطة العلاج ...

🔸️ بنبدأ بال ULT بعد اسبوعين من ال Flare

🔸️خلال ما بنبدأ ب ULT بنعطي Prophylaxis خوفا من انه يحصل Gout flare
( لان اغلب ادويه ال ULT هتقلل ال Uric acid بالدم ،، بالتالي هتصير ال Uric acid crystal تتحرك من ال joint وتطلع ع الدم ، حركة الكريستالات هتعمل Irritation وتدخل المريض ب
Acute gout flare )
افضل خيار لل Prophylaxis هو ال Colchicine
بنعطي حبه مره او مرتين باليوم
Colchicine Dose :

🔸️normal kidneyfunction : (0.5-0.6) mg once or twice daily

🔸️In CKD Stage 3 : (0.5 -0.6) mg Onc daily

🔸️In CKD Stage 4: half of a 0.5
or 0.6 mg tablet daily (if tablets can be split) or one 0.5 or 0.6 mg tablet every two to three days

⛔️Colchicine should not be used in patients with stage 5 CKD undergoing dialysis for end-stage kidney disease.

بنستمر عليه أول 3 ل 6 شهور من بداية ال ULT
ولو المريض عنده Tophi بعض الدراسات بتفضل تستمر بالكولشسين لمدة 12 شهر من بداية اعطاء ال ULT ...

لو المريض عنده م يمنع استخدام ال Colchicine بنعطي بدلا عنه low dose NSAIDs زي مثلا ال Naproxen
بجرعة 250mg مرتين باليوم ..

🔸️لو المريض عنده م يمنع من استخدام الكولشسين
وال NSAIDs فبنلجأ لل Steroid
Prednisolon: 5 -7.5mg daily ..
